Meningeal involvement during multiple myeloma (MM) remains rare and often underdiagnosed. Non-infectious aseptic meningitis occurs in less than 1% of all MM and typically complicates cases already diagnosed and treated for this malignant hemopathy. The inaugural forms are exceptional with only a few sporadic cases previously reported in the world literature.
We report an original observation of noninfective aseptic meningitis as an initial and isolated manifestation revealing stage IIIA IgG-kappa MM in a 68-year-old woman, with favorable evolution after two cures of anti-myelomatous poly-chemotherapy.
As rare as it is, this clinical presentation of multiple myeloma deserves to be known by all healthcare professional because of its important prognostic implications.

Key words: Multiple myeloma, Aseptic meningitis, Myelomatous meningitis, Meningitis
