Takayasu's arteritis is a rare, chronic progressive panendarteritis involving the aorta and its main branches. It is one of the most important causes of renovascular hypertension. Renal autotransplantation is an unusual but safe procedure that implies a combination of living nephrectomy and standard renal transplantation in the same patient.
A balanced anesthetic technique, maintenance of stable perioperative hemodynamics with maintaining renal perfusion pressure is required for a successful outcome. we describe the successful anesthetic management of a patient with Takayasu's arteritis and severe hypertension refractory to medical management requiring renal autotransplantation.
